Form: D4 (DVLA D4 Group 2 Medical Form from GOV.UK), plus a UDC Licensing Services Medical Certificate
Worth double-checking: Uttlesford uses the DVLA D4 rather than a bespoke Group 2 form, and requires the driver to hand the GP a Summary Care Record - so the assessing doctor must be working from the records even if not the driver's own GP. Wording says 'your GP'.
