Who can carry it out

Your own GP, or a doctor with your records

Your own practice is the straightforward route, but another doctor can do it if they have had full access to your medical history. Private clinics that request your records can qualify. One that examines you without them cannot.

What Bradford requires

Must be completed by a registered GP who has had full access to your medical records.
If it is not your own GP, they must confirm they have had access to your full medical history.
Dated within six months.
Sections 2 and 4 must be completed in the presence of the GP doing the report.
