What Pendle requires

“THIS MEDICAL MAY BE COMPLETED BY YOUR OWN GENERAL PRACTITIONER WITH WHOM YOU ARE REGISTERED WITH WHO HAS FULL ACCESS TO YOUR RECORDS OR A QUALIFIED MEDICAL PRACTIONER WHO HAS SEEN A SUMMARY OF YOUR MEDICAL RECORDS”
How long it lasts: 4 months (council FAQ: 'How long is my medical valid for? 4 months'). The form itself does not print a validity, so drivers relying only on the form would not see it
How often: On first application (all ages), then at ages 30, 40, 50, 55, 60, and every twelve months thereafter. UNUSUAL - Pendle is the only council in this batch starting at 30 and with a 40 step; the gaps are 10 years then 5 years then annual from 60. The examining doctor can also override on the form: 'I CONSIDER A MEDICAL IS REQUIRED IN ____ YEARS TIME DUE TO AN EXISTING/NEW MEDICAL CONDITION'
