Taxi driver medical in Lancaster

Which doctor Lancaster will accept, how often you have to repeat it and which form they want. Councils disagree with each other on every one of those, so this is Lancaster’s answer and nobody else’s.

What Lancaster requires

“Lancaster City Council has adopted the DVLA ‘Group 2’ medical standards of fitness, this is the standard applied to all Private Hire and Hackney Carriage drivers, upon first grant of a licence and renewal”
How often: D4 medical examination on initial application. Then on each 3-year renewal a self-declaration only, until age 45 when a further medical examination is required; self-declarations then for a further 3 years; from age 65 an annual D4 medical examination
Form: DVLA D4 for the examination, plus the council's own Medical Declaration / Statement of Fitness form at renewal. The older application pack also carries a council-branded 'Medical Report on an applicant for a Hackney Carriage or Private Hire Driver's Licence' form
Possible exemption: Not published – no HGV/PSV/PCV or Group 2 licence exemption stated
