Taxi driver medical in Castle Point

Only a provider the council names

The council will not accept a certificate from a doctor of your choosing. Use the provider they name, or you will have paid for a medical twice.

What Castle Point requires

“The following medical providers are approved by the Council to undertake a medical examination to the group 2 standards for licence holders and applicants: a) The GP surgery at which the applicant /licence holder is registered; b) Cotswold Medicals Ltd; and c) Where any of the above options are not available, any medical provider with the written consent of the Council’s Licensing Manager”
How often: On first application, then every 5 years until you are 65. From 65 it is annual, and it must be done within the 21 days before your birthday
Form: A D4 medical report and certificate on the D4 form provided by the Council, to Group 2 standard, by a practitioner registered with the General Medical Council
