Taxi driver medical in Broadland

You are not tied to your own surgery, which usually means a shorter wait and a lower fee. The condition is the records: the doctor must have seen your full history, not just examined you on the day.

What Broadland requires

“A copy of the medical assessment will be provided to you once you have completed the pre-application form (do not use an existing medical assessment which has previously been provided to you)”
How long it lasts: No maximum age of certificate is published. What IS published is a bar on reusing an old one: "do not use an existing medical assessment which has previously been provided to you" - the council issues a fresh form after the pre-application. (For context, the pre-application reference number itself "is only valid for six months from the date of receipt".)
How often: At every new and renewal application - and licences renew every three years, so effectively every 3 years. From age 65, annual medical assessments
Form: The council's own medical form, written to DVLA Group 2 standards. Issued to the applicant by the council; not the DVLA D4
