Taxi driver medical in Caerphilly

What Caerphilly requires

“Sections 1-10 of this medical report must be carried out by the applicant’s own general practitioner (GP) or another GP at the applicant’s registered practice that has full access to their medical records”
How long it lasts: Valid for 4 months from the date the examining doctor, optician or optometrist signs it
How often: On application, then every 5 years between the ages of 45 and 65, then every year when the driver is aged 65 or over. Also whenever requested by the licensing authority or the medical practitioner
Form: Council's own – the CCBC medical form, 'medical report on an applicant for a hackney carriage or private hire vehicle driver's licence', described as equivalent to the DVLA Group 2 medical. Sections 1-10 by the GP; the vision assessment may be done by an optician, optometrist or doctor
Possible exemption: Not published – no HGV/PSV/PCV or Group 2 licence exemption stated
