Taxi driver medical in Havant

• “Please note: You must ensure that you take a summary of your medical records to your appointment if you have your medical report completed by D4Drivers. Failure to do so will result in having to reschedule your appointment at an additional cost. A summary of your medical records can be obtained from your GP on request, or downloaded from the NHS app”
• How often: Prior to a licence being granted (initial application); at 45 years of age and every five years thereafter; at 65 years of age and every year thereafter
• Form: Council's own medical assessment form - 'using the medical assessment form provided by the Council'. Listed on the website as 'Medical Report Form for Drivers'. Assessed to DVLA Group 2 Standards
• Possible exemption: Not stated - no HGV/PSV/PCV or DVLA Group 2 licence exemption is published. (The 'exemption certificates' in this policy are Equality Act 2010 exemptions from assistance-dog and mobility-assistance duties.) NOTE: the policy's reference to Group 2 being 'as applied by the DVLA to the licensing of lorry and bus drivers' is only explaining the standard, not granting an exemption
