Taxi driver medical in Coventry

Which doctor Coventry will accept, how often you have to repeat it and which form they want. Councils disagree with each other on every one of those, so this is Coventry’s answer and nobody else’s.

What Coventry requires

• “The medical is carried out by any Medical Practitioner You will need to ensure that you collect a 'Certificate of Fitness 009' from the Taxi Licensing Office or at the back of this information document and ensure that your Medical Practitioner completes it and hands it back to you for you to return it to the TLO”
• How often: CONFLICTING. Driver Information Document and the 2026-2031 policy both say: on first application, then 5-yearly until 65, then annually. The council's own medical form TLO/009 says: on first grant, 5-yearly up to 45, then 3-yearly up to 65, then annually. The form also lets the examining GP set an earlier review date which overrides the standard frequency
• Form: council's own
• Their own sources disagree on this. Coventry is unusually permissive on WHO: 'The medical is carried out by any Medical Practitioner' and 'Applicants must arrange for any Medical Practitioner' - no requirement that it be the driver's own GP and no requirement to see the medical records, only that the examiner 'wher Ring them before you book anything.
