You are not tied to your own surgery, which usually means a shorter wait and a lower fee. The condition is the records: the doctor must have seen your full history, not just examined you on the day.

What Wolverhampton requires

The DVLA D4 form, completed in person by any GMC-registered practitioner who has access to your full medical records. It does not have to be your own GP.
Since 1 January 2025 all eight pages must be submitted, fully complete, with your name and date of birth on every page.
Valid for applications made within four months of the examination.
Assessed to DVLA Group 2 standards.
