Taxi driver medical in Lichfield

What Lichfield requires

• “When a Group 2 medical is not required at renewal, you must complete a self-declaration form with your renewal application to confirm that there have not been any changes to your health since the last medical examination”
• How often: When you first apply, at every renewal after you turn 45, and at 65 and every year after that. A doctor can also decide you need them more often. Existing drivers under 45 submit one at their next renewal, then not again until the renewal on reaching 45
• Form: The council's own medical form, downloadable from the apply page - plus a separate self-declaration form for renewals where no medical is due
