Taxi driver medical in Slough

What Slough requires

• “Any medical form completed by a private GP practitioner, must be accompanied by a declaration from the GP stating that they have had full access to the applicants medical records”
• How long it lasts: 3 months, measured to the GRANT date not the application date - 'For new applicants, the medical certificate should be no more than 3 months old at the time the licence is issued.' This is one of the councils where a slow application can expire the medical
• How often: every 5 years from age 50, then annually from age 65 (annually at any age for insulin-dependent diabetes)
• Form: council's own (the Council's prescribed form, provided by the Licensing Team)
