Which doctor North East Lincolnshire will accept, how often you have to repeat it and which form they want. Councils disagree with each other on every one of those, so this is North East Lincolnshire’s answer and nobody else’s.
Only the practice you are registered with can complete this one. You cannot shop around for a cheaper medical, and if your practice is slow to offer appointments that is the timetable you are stuck with, so book early.
