What Luton requires

“To submit an application for private hire and hackney carriage driver’s licence, you will need to meet ALL supporting documentation required, this includes a DVLA Group 2 medical standards medical certificate completed by your GP”
How often: A Group 2 medical certificate completed by your GP is required with the application. Licences run three years as standard, so the medical comes round with each renewal. From the age of 60 it becomes ANNUAL - condition 14(c) requires a certificate on the grant or renewal of the licence each year. Luton's 60 threshold is earlier than the 65 most councils use
Form: Luton's own Certificate of fitness to drive a hackney carriage or private hire vehicle, completed by your GP against DVLA Group 2 standards. The doctor is asked to confirm on the form whether they had access to your full medical records
