Taxi driver medical in Bolsover

“When you attend your appointment you will need to take a medical declaration and group two medical form and ask your doctor to complete all sections”
How often: At first application, then every five years up to age 65; from 65, annually. The council may also demand a further medical at any time where it doubts fitness
Form: The council's own pair of forms - a 'medical declaration' and a 'group two medical form' - handed to the applicant on completion of the knowledge test. They are not published for download anywhere on the site, so a driver cannot get them in advance
Possible exemption: not published. The Driver Policy applies the DVLA Group 2 standard but offers no exemption for HGV/PSV/PCV holders and none is mentioned anywhere on the drivers page
