Taxi driver medical in Swindon

Which doctor Swindon will accept, how often you have to repeat it and which form they want. Councils disagree with each other on every one of those, so this is Swindon’s answer and nobody else’s.

What Swindon requires

• “3. I confirm that the information provided is a true and accurate account of the applicant's health as known to me and that the applicant is registered at this medical practice, or I have had sight of their full medical records when completing this form”
• How long it lasts: 4 months - 'This report must be submitted to the council within 4 months of the doctor signing the report' / policy: '...within 4 months from the date of examination'
• How often: Required from all new applicants regardless of age. Existing drivers: every three years from the age of 47, then annually from the age of 65. UNUSUAL - the trigger age is 47, not the more common 45. Council can recall a licence holder for a medical at any time; more frequent checks if the medical practitioner thinks it necessary. Insulin/sulphonylurea/glinide diabetes: normal age-based interval plus a specialist medical before the licence is issued and annually after
• Form: council's own

Taken from Hackney Carriage and Private Hire Vehicle Driver Medical Report form - front page and 'What you (the applicant) have to do' / 'What the Doctor has to do'.
