Taxi driver medical in Cherwell

Which doctor Cherwell will accept, how often you have to repeat it and which form they want. Councils disagree with each other on every one of those, so this is Cherwell’s answer and nobody else’s.

What Cherwell requires

• “Before we can deal with your application, you must have had a Group 2 medical examination. This must be done by a doctor who has access to your full medical records; this will usually be your general practitioner. They will need to examine you and fill in a DVLA D4 medical questionnaire”
• How often: On first application, then at each renewal of a 3-year licence, until age 65, after which an annual medical fitness assessment is required. Concession: someone who applies at 64 and turns 65 during the first year does not need another medical until renewal. Insulin-treated diabetics need an ANNUAL diabetes-specific report on top of everything else
• Form: DVLA D4. The council explicitly says 'fill in a DVLA D4 medical questionnaire' and 'you must submit to us the full completed D4 medical form'; the renewal guidance calls it 'your completed D4 Group 2 Medical Form'
